Spoke with Verizon. On the FIOS side, they said it was for new customers only (rats - only signed up last month), and for the wireless side, they said existing customers should see it in their Verizon Up section and should have been sent a code in an email, per their IM team "That is for existing customer since a promo code has been shared with our customers over the email."

So if you're a wireless VZ customer you should be good. If you're a FIOS only customer, move on. If you want to buy FIOS, you should ask about it as an incentive - no guarantees.
